For daycare and childcare providers

Daycare Receipt Template for Childcare Providers

Families ask for receipts month after month and again at tax time. Writing each one by hand takes time, and inconsistent formats make your records harder to follow.

Create one clear receipt for the care you provided and the payment you actually received. Add the child, service period and provider details, then print or save a PDF for your records and the family.

Receipt details

What should a daycare payment receipt show?

A daycare payment receipt should identify the provider, family and child, describe the care, and show the service period and payment actually received. Add a payment method or receipt number when it helps distinguish the record. This tool leaves the provider tax ID field optional.

Service dates

How to record a childcare service period

Enter the start and end dates covered by the payment so the family can match the receipt to the care provided. For weekly or monthly care, use the dates for that specific period; leave the dates blank until you know what the payment covers.

Example

What does a completed daycare receipt look like?

The sample above includes a fictional provider, family, child and service period. Its payment date and received amount are visibly left as placeholders; enter those only after payment is received, because a receipt records a payment rather than money still owed.

FAQ

Frequently asked questions

Can I use a daycare receipt for taxes?
For Form 2441, families generally need the provider’s name, address and taxpayer identification number (TIN). This receipt can help them keep a record of care and payment, but this tool’s provider tax ID field is optional. Check the specific requirements of the IRS or your plan; this tool does not guarantee tax deductibility or reimbursement.
Should I use the amount due or the amount received?
Use the amount you actually received and the date you received it. A receipt records a payment; it is different from an invoice requesting payment.
Can I save a daycare receipt as a PDF?
Yes. Fill in the receipt, review the preview, then use your browser’s print dialog to print or save it as a PDF.
Where can I get a free printable daycare receipt?
Use the online receipt maker to fill in the care and payment details, then print the receipt or save the completed page as a PDF in your browser. If you prefer to write it by hand, download a blank daycare receipt as a PDF or Word file from the download section below the receipt maker.
Can I get a daycare receipt in Word or PDF?
Yes. You can download a blank daycare receipt as a Word or PDF file and fill it in by hand or in Word. Or fill in the online receipt maker, review the preview, then print it or save it as a PDF from your browser.
